Driving to Games and Practices

Just a reminder - students are not permitted to drive other students to games or practices. All drivers (including student drivers who drive only themselves) must complete the necessary documentation - see Driver Documents and Group Travel Form below.

Driver Documents

Please complete the Form J and include a copy of your license, motor vehicle inspection, and insurance policy (the policy page that shows coverage and not the insurance card).

Please drop the paperwork off at the Park View office.

Group Travel Form

Just a reminder to all drivers that Devan must receive the list of vehicles, drivers, and driver cell numbers (if applicable) to complete the Group Travel Form and Principal's Check List. The sooner we have the information the better! We cannot travel unless the form is complete.

We must complete the form for every road game. Students who drive themselves must also submit their information for the form.
